Can TRT shrink your testicles?

Yes. When LH falls, the testes produce less testosterone and often shrink somewhat. The change is usually more noticeable to you than to anyone else, but it should be mentioned before you start so it is not a surprise.

hCG is sometimes used to support intratesticular testosterone and volume when that is a priority. It is a prescribed add-on, not a research-peptide experiment.

Does size return after stopping?

Often partly or fully as gonadotropins return, with the same caveats as fertility recovery — variable and not instant.
