TRT and Fertility: Does Testosterone Replacement Therapy Lower Sperm Count?

Does TRT lower sperm count?

Yes. Exogenous testosterone typically suppresses pituitary LH and FSH. FSH support is important for spermatogenesis, so sperm counts often fall — sometimes to azoospermia — while you remain on TRT.

This is expected, not rare. Men who want to conceive in the near term are usually offered a different plan: observation, lifestyle, clomiphene/enclomiphene, hCG, or gonadorelin, depending on the diagnosis.

What if I might want children later?

Tell your Vita Bella clinician before you start. Options can include delaying TRT, using fertility-preserving medications, banking sperm, or combining strategies. Waiting until a partner is pregnant to mention fertility is a harder recovery conversation.

Stopping TRT does not guarantee a normal semen analysis on a fixed timetable. Recovery can take months and is not certain in every man, especially after long-term suppression.

Can hCG or SERMs protect fertility on testosterone?

Some protocols add hCG (or discuss gonadorelin) to maintain intratesticular testosterone while a man is on TRT. SERMs are more often used instead of TRT when the goal is to raise endogenous testosterone and preserve sperm. These are individualized and not DIY add-ons from a research peptide site.

Does TRT cause permanent infertility?

It can cause prolonged suppression. Many men recover sperm production after stopping, but it is not guaranteed or fast. Plan before you start.

Should I get a semen analysis first?

If pregnancy is a near-term goal, a baseline semen analysis is often useful. Ask your clinician; do not assume TRT is compatible with trying to conceive.
