Duration is a shared decision, not an automatic lifetime sentence.
There is no universal stop date. Men with irreversible primary or secondary hypogonadism often remain on monitored replacement indefinitely, the same way other hormone deficiencies are replaced.
Men whose low T was driven by obesity, sleep apnea, or a reversible drug may try a supervised pause after those issues improve. Long-term safety is about hematocrit, prostate surveillance as indicated, cardiovascular risk management, and follow-up — not a 2-year expiration stamp.
The prescription can be stopped. The underlying deficiency may still be there. “Permanent” should mean “we expect ongoing replacement unless something changes,” not “you can never get off.” Fertility plans, side effects, or a change in diagnosis can all reopen the question.
