What Happens When You Stop TRT?

Axis recovery, symptom rebound, and why stopping should be planned.

What happens when you stop TRT?

Exogenous testosterone clears over days to weeks depending on the ester or gel. LH and FSH may then rise as the brain tries to restart testicular production. During that gap, libido, energy, mood, and sense of well-being often dip — sometimes below your pre-TRT baseline for a while.

Sperm production, if it recovers, lags behind symptom changes. Hematocrit usually drifts down. Do not interpret the first miserable fortnight as proof you must restart at a higher dose without a check-in.

How should you stop?

Plan it with your clinician. Some men taper; some switch to a SERM or hCG bridge when fertility is the goal. Abruptly stopping a high dose after years of use is more likely to feel brutal than a supervised transition.

If you stop because of a side effect such as high hematocrit, the stop is still a medical event — get the follow-up labs.

FAQs

Will my testosterone go to zero if I stop?

It falls as the medicine clears, then your testes may resume production. How high it returns depends on why you were hypogonadal and how long you were suppressed.

Can I restart later?

Often yes, after a fresh look at labs and goals. Restarting is still a prescription decision.
